Open the HZS_System database in SQL Server Management studio with an administrator account and open the HZS_User table by right clicking as choosing "Edit Top 200 Rows"
Then find the lowest UserID record which will be the administrator account that was created when QES was first run.
If the username was lost replace the UserName with rgzJJDblSN0=
If the password was lost replace the Password field with rgzJJDblSN0=
Both of those values are the encrypted vales that translates back to lowercase "sa"
Log back into QES and open the change password form under the System tab and change the sa users password to something more secure.
